The Wellness Profile is your personalized health snapshot, combining two important components. Together these steps give you a clearer picture of your health and help guide lifestyle choices.
✅ Step 1: Complete the Biometric Screening
Option A: Worksite Screening
Get your annual biometric screening through a take-home kit, at Quest Patient Center, or Worksite Location. The screening will measure A1c, total cholesterol and triglyceride levels, blood glucose, weight, and blood pressure.

Option B: Visit your personal Doctor
Can't make it to a biometric screening? No problem. You can visit your own doctor for an annual physical and submit the form to verify completion. We do not collect the health data from your physical.
Both the submittal form and acknowledgement form must be signed and submitted to the Wellness Program at wellness.program@ventura.org.
